Dear Parent/Guardian of your Manning student(s),
If your student is new to Jefferson County Schools or you’re a returning student you will be required to provide a copy of an up to date immunization record at the time of registration. If you are unaware of the immunization requirements you can contact the Colorado Department of Public Health and Environment.
Regarding other health care needs, parents should bring any special health concerns to the attention of the clinic aide as soon as possible. All medications that need to be taken during the school day must be brought into the clinic by the parent/guardian accompanied with an authorized medication agreement, (form 924). You can obtain this form from our web site. This form will need to be signed by the parent/guardian for all over the counter medications and the student's physician will need to sign for prescription drugs. The Manning School no longer provides Tylenol, Aspirin or Ibuprofen for the students. Should your student use these medications they can choose two options. The first option would be to carry a daily dosage of Tylenol, Aspirin or Ibuprofen with them as long as it’s in its original container. Our preference would be a prepackaged daily dose with an expiration that lasts through the school year. Your second option would be to bring in the container for the student to be left in the clinic for the school year.
Students with asthma may carry their own inhalers with them at school; however they must complete a Medication Agreement 924 form along with a Contract for Students Carrying Inhaler with them at School form. Should you also choose to have a back up in the clinic that too is highly encouraged.
Students with Allergy and or Anaphylaxis need an action plan in place as well. If an Epi-pen is necessary please keep in mind the expiration date when dropping it off in the clinic. This date should ideally not expire until the end of the school year.
